The router ran away on me so I had to chisel out this square and insert a repair. After the glue dried, I used a pull saw and a jack plane to flatten. Of course, when I hit it with the plane a small piece tore out and then I had to repair that as well. Other than getting a better match on the grain, how can I make this more seamless?

And in my partial defense, the grain match was much better on the top side of that piece before I cut it off.
To get better "seams" I would guess the most useful thing would be a paring block. You need nice crisp edges in your mortise or you'll see this little gaps.
And a paring block is possibly the easiest way to improve the mortise walls.

Another tip is to slightly make the patch bigger and bevel the patch edges in so it's so very slightly wedge shaped
