If it's rough, then yeah, you need to sand more.
You may have a low spot that is causing your sanding efforts to miss that spot.
is this a commercial butcher block? If so, ensure that it's solid, not veneer
If the surface is very uneven, I would start at 80 grit, sand with the grain and put the sandpaper on a block to keep the surface flatter.
Then 120 grit and then 180 gritand then put your finish on.
Depending on the depth of the imperfection, you might be in for an awful lot of sanding.
