Sometimes get "You found a hidden vein!" message with no follow up saying which ore, get it more often when mining in areas with little material nearby, could be because it tries to spawn an ore in air.
Ore Spawn plugin forgets blocks were placed by a player after some time (or maybe after player relog or server reset).
