so something something scheduler is causing this

That post is veeery old. It was about the old v1 scheduler. Recent versions of Anki don't support the v1 scheduler any more, and even if you are on v2, I think they now force you to v3.
Okay still can be a scheduler problem or add on issue with the scheduling helper?
Are you sure it's the same card, and not a sibling?
How do you review your cards? Anything unusual? Multiple devices? Or maybe filtered decks?
On such a card where you are really sure you've seen it before, could you maybe share the card info?

i do use multiple devices, yes
that's the only unusual thing
Siblings are cards created from the same note.
That might be the issue. Do you make sure to sync before and after your review session? Anki Desktop by default syncs on startup and closing, I'd assume that any mobile versions sync manually by default.
i do manually sync every time, if i didn't do that weird stuff would start happening
when i edit a card, will the changes appear on the sibling?
if the answer to that is yes, it could mean that i have only been looking at siblings the entire time
yes, they force to v3. you cannot change to v2 anymore since 23.10, i think
Depends on what you mean by "edit a card", as that's not a thing.
You can edit the note. That's where the fields are. Editing a note affects all cards made from it.
And you can edit the card template that's used to generate the HTML for the card. Changing the template affects all cards of that type.
