If you don't mind, I will just copy the comment I posted in Gone Rogue channel.

I think I know what happened. In Descent the boss is removed. But not in Craig. I got Garm, killed him, SOMEHOW the quest for killing the boss was finished in the middle of the route, I got ported to town. Picked up the next kill the boss quest, entered the portal and got back to my run which was still in the middle of the run. I killed the boss, the SECOND quest was flagged completed, but there was no portal. If I had to bet, this false quest completion has something to do with the portal not opening on the boss.
