I went in with my friend who wanted barrow dyad and got soft locked twice in a row at the boss. Should wait until reset to see if Bungie fixed this

After you kill the boss, his taken form jumps down and starts forming a blight which triggers door closing sequence and end of the mission. It was a bug when this mission launched with heresy that taken boss won't appear but was fixed. Seems like this bug is back for some

I don't know about this ops specifically but for encore and kells fall, exotic only dropped from A or better ratings. It was still RNG but most people got it within 5 runs
