It happened to me, but managed to meditate, just keep trying and hopfully it works!

restart game talk to her exit switch to prismatic talk to her again exit meditate

you have to talk to her from her left not next to meditate and dont be super close to her be like at the edge of the pop up

I tried relaunching and meditating again for like 30mins wouldn't work
