Im on step 2 on my second character. I try to input the code, which is Witch Queen, Enter, Pyramid and it gives me the Wrong input animation and doesnt progress the quest. Anyone have any suggestions?

This is a widespread bug. There is a mismatch in the game how certain element are account wide and others are character bound. For example, the "Initiate" level, which is the source of the issues imo ; on your 2nd character, he must already be "Third level Initiate".
Only solution for now is to find someone who is on his 1st character on that same step, have him enter the code, which will enable you to progress your quest. You'll then need someone similar for step 10 (I'm stuck there now lol).
Bonus bug for me, I can't focus gear even on the character I completed the quest on (as I remain 3rd Level Initiate and didn't become "Fully Initiated" as I should've), but still, I was able to get the class item and quest completion on Warlock, yet couldn't focus from that point on.
