To get the harbinger title, i need to do 40-50 runs of alters of summon to get the shell. Do i need to do Tier V + Boss to count as a full run, or can it just be a bunch of Tier 1's?

I'm pretty sure it's waves and not full runs. Either way I would test and check if the boss fight counts, because on the moon bounty requiring you to do altars it doesn't count the final bossfight
One run gets you to 90% or whatever, and you would have to do tier 1 again for the completion
