I have killed all bosses according to triumphs, all collectibles, and the placing them in the vault workaround didnt work. Help?

The taken eyeball in the Blooming is the one that has it as a random drop, likely that's the one missing, and the triumph tracker is not accurate, it's counting all kills against them as one of each boss. You'll have to either farm the eye or remember which ones you've faught then fight the one that was missing
