I have had rare occasions where the back wall will not show symbols yet the player in question had 2 knights up, so therefore had 2 symbols at least in bank (it was their key symbols, we had completed passing.
confirmed at least once via recording, and reported a couple other times

you keep your perks but if you kill a knight mid it can bug out

I've had this same issue and it isn't a rare occurrence at all. I'm not sure but I think it has to do with the wall receiving status effects and AoE damage
Like if you throw a grenade at it or it gets hit by Stasis Shatter or Solar Ignition, it just stops working
