#Action Figure Glitch

Ive collected 4 action figures on Neomuna but i can only place 3? Going orbit or restarting game didnt fix it, Double checked all 4 figures.

I'm having a glitch where I can't collect the 4th. I've done the activity at the bar like 15 times until it says "a secret is revealed" and the action figure doesn't spawn in. Can't find any info on this problem
