I know there are some secret challenges to have the best rewards possible. Including kill every enemy/champion, complete a certain time... is there anything else I am missing? Does time increase by difficulty too?

Just kill all (sometimes you can skip one or two) champions.
If you wait too long, eventually your score will start draining, but that doesn't effect loot afaik.

Is this referring to champions in the boss fight, like right before a boss dies, they wanna drop a champion
