Weapon disappearing from vault

I lost my roar of the bear from the vault. I checked all characters and it has simply disappeared. Does anyone know how to fix this?

there aren't really anyways to "lose" items, unless you got error coded as it dropped and the progress didn't count which this doesn't sound like.
