How do I unlock each individual perk for the revision zero ? (not the catalysts or hunters trace)

How about getting the achievement for having the catalyst? I have gotten all three “catalyst” perks but still haven’t unlocked that achievement.
Maybe it’s four catalysts

That is kinda bugged right now. Craft a new Revision Zero with no perk and it should unlock @eternal thunder
