I'm not a console gamer so I am not sure.
On PC you just do /join [Name][####] to join people.

Next time you wanna join on someone just type their code they post in a chat.
Might be harder for you guys since you can't copy/paste like PC users but yes. That's it.

For instance mine would be /join Just Monika#1264
You'd type it in exactly like that, and bam. Joined the player.
Don't actually type that though. Am not playing. GL you guys.
