The first encounter of spire should drop helmet, arms, boots, auto, scout and lmg. I've done 58 runs and only ever had the arms, boots, scout and auto. despite having got the lmg from the boss previously.

It is possible to farm for new items, as I farmed the final boss 10+ times for the bond on warlock. It's just rng.
Just make sure you've done a full run on the character first before you farm, I know that was a requirement for the other dungeons to get new items.

I've done a full run on titan, and now 62 runs of 1st encounter
no hat or lmg, but i got the lmg previously from boss
so 2/6 bits of loot not dropping at all

In the week you want to farm, do a full run first. In previous dungeons not doing a full run first would just give you the loot that you got from that encounter.
So if you only did the first encounter and got the scout, all you would get from farming would be the scout. At least that's how it worked with grasp and I think duality.
Your rng is just extremely unlucky then. I was able to get all possible drops on my warlock the first week through farming.
