Overall, the chart was fun to play and enjoyable.
It is clearly well structured and well thought out, however I feel like the greatest weakness of the chart in is current state is that it feels a bit too cut and paste.
It would be great if you could play around with the variation in jack patterns, especially since you seem to restrict yourself to either [12] or [34] 16th jacks in the chorus.
