I feel like I once found the explanation of what this is and why, but is this correct, and can someone let me know under what circumstances it happens? Thanks!

I would assume it can be deleted once it is no longer provided, do you have an example of when this is not the case?

I'll try to find one. I might be wrong about the one I was looking at—it looked like it wasn't provided because there were two copies of the same entity (firmware update), but the bad one is unavailable, not not provided. So it may be a bug in the esphome stack providing an unavailable second copy. Will follow up or delete this once I look more.
