Hello,
I've an home assistant yellow with enabled thread (no multiprotocol) firmware/support.
The yellow is configured with dual stack (ipv4 & ipv6).
My new OpenThread Boarder Router is configured inside the integration to be my preferred network, inside the companion app on iOS, I've "send credentials to phone" & "send credentials to home assistant" for my thread integration.
The addons Matter Server and OpenThread Boarder Router are installed, up and running.

When I attempt to add a new tado X thermostat (after resetting the device to manufacture defaults once again) with the home asssistant companion app at "Devices => Add new matter device". I scan the QR code and keep my iPhone near the thermostat. The Thermostat itself says "connecting..." and after a few seconds even shows "connected". But at the same time my homekit (?; see screenshot) pops up with an generic error message "device could not be added" (roughly translated from german "Gerät kann nicht hinzugefügt werden"). Down below it reads "Home could not establish a connection to this device". My Homekit setup was initialy only Home Assistant Homebridge. But I tried to delete my "Home" and added it once again with an AppleTV as my main bridge and homekit bridge to integrate all my home assistant devices without any success to fix this problem.

My network is flat with a fritzbox soho router/access point providing WiFi 2.4GHz. Multicast in general seems to be fine, since i can resolve in example mDNS queries like "homeassistant.local".

Sadly there are no related logs in the home assistant protocol (Matter or OpenThread) and i suspect an issue with my apple device. I tested to reset all my settings with this wipe profile (https://www.reddit.com/r/HomeKit/comments/yy2oy7/apple_has_a_downloadable_profile_to_really_clear/), but without any success. The issue can also be reproduced with an secondary iPad Mini inside the same network. Any advice is much appriciated. Thanks!

I could "fix" my issue by using an android phone with the companion app instead of my iphone. The setup worked like a charm. It's still a mystery to me why iOS does not work. But hey, it's a workaround.

out of curiosity, which iOS version were you using?
