Hi! Just to remember about this problem @cerulean igloo .
Looking into translations, the word "adicionar" is not even in pt-br sentences or responses files, but in 'pt' language. I think Assist is mixing both languages.
Here in Brazil it's 'pt-br'......only 'pt' is for Portugal.

I've tries this with local engines (piper and whisper), same problem and same error.

voice components (STT/TTS) are not even relevant for this. we will debug it with text only.
please provide the following, so that i can try to reproduce:

the used pipeline settings
the pipeline debug log (https://my.home-assistant.io/redirect/voice_assistants/ > select an assistant > top right > Debug) - paste the big raw output at the end
