Oh, weird. It has been a few days, but I just went through the process again. I thought I could read and modify with the app, but that is not what happened this time so I might have misremembered. My steps:

  1. Write a tag from Home Assistant app on Android
  2. Open NFC Tools app and read the same tag
  3. Scroll down to find Record 1 which should be a home-assistant.io URL
  4. Tap on the record and copy to clipboard
  5. Go to the write tab and add a record
  6. Select "Custom URL" and paste the URL you copied
  7. Tap to add another record
  8. Select Application then click on the Android icon on the right and scroll down to select the Home Assistant app
  9. Hit Okay then tap the Write button to write the new records to the tag

Hard to say what happened to the tag that caused it to no longer work or if there is a way to fix that tag.
