Inclusion failed tends to be a wireless connectivity issue

Usually goes away for me when I plug it into a USB extension

Hmm, I tried it both on the extension, as well as without it, and both times same result.

Otherwise its pretty much impossible to find out why it is failing, which leaves try and error
