Spoiler log wrong?

Hey all. Have an issue with a seed where the supposed Mushroom reward is supposed to be the Lamp according to the log but it wasn't and I can't conceivably think of any other checks. Has this ever happened to anyone? Yes, I triple-checked the name on the file and it's for the right seed, all the other items i've found so far match up

In the spoiler log, "Mushroom" means the location in the lost woods where the mushroom would be in the vanilla game and "Potion Shop" is the witch's hut where you get the reward for turning in the mushroom.
