The stand damaged the nurse because his body was still inside her even when he got punched. As you can see in the scene, the Green stand can make its body become some tentacles.

Avdol died after a day after the Dio battle.
When swallowed by ice into an alternate dimension, Avdol had stuff to eat. He could've made some fire and cooked polnareff's feet that got iced and at least had somewhere to survive and cooked it. But then after starvation, in the alternate dimension Avdol dies.
Now in the OVA, Avdol also had Iggy's half and Polnareff's foot to eat. Although he didn't have any legs, he can burn it on the ground and cook it. Have something survive for two days, just cut a piece of the iggy half and then eat it later yea...
