yep, same thing happened to me
I assume what's happening is that the hat gets upgraded every new game +, so since it deletes the previous version in order to create the upgraded version, if you're wearing it when changing new game +, for some reason the headpiece slot defaults to random clothing
therefore making Kyle headless
