yup, unless I mistake the location you talk about. but I am certain you speak off the axe that is hanging above the exterior doorway of a balcony in a house in the woods.
that is tied to a sidequest as far as I know.

just keep progressing the main story until you it unlocks.
sidequest is about two brothers. one of them is sick and needs meds.
you can already find them in the cave beforehand and they do have some dialogue about him being sick.
not sure if you also have to do any other sidequest to unlock it, since I did all of them when I got them.
