So im trying to get 100% in this game and im stuck on the trophy where you gotta max out your HP i have collected 123 /126 but i cant find the last 3

as i far as i know this achievement needs a ng+

you have to look for that ng+ inhibitors

There are 126 Inhibitors in the base game, with an additional 30 being obtainable in the NG+

There are YT videos showing the secret ones, which are not visible on the map

There are also some that are bugs and you can’t get it until you go into new game plus or join someone else’s game
