unknown locations "?" and certain mission progress not saving

As a completionist id like to be able to 100% absolutely everything.
however some mission progress will not save such as "double time" side quest.
half of all "?" unknown locations are not saving even after visiting and interacting with said unknown location. is there anything i can do about this?
