You should report it everytime you see it. Somehow some bad eval gets saved and a) dev's can delete the eval and b) maybe figure out if there is still something causing this, or if this is just remians from an already fixed bug.
Screenshots are good, since I could not reproduce the issue as my default pv is more then 1.
