Here's a fun one. Equipment is attached to a creature and in the graveyard at the same time.
I attempted to attach [[Krovod's Haunch]] to my wolf token.
AI cast [[Nature's Chant]] in response, destroying the equipment. (Side note, inadvertent brilliant play by the AI, as waiting to do it in response means I can't spend my mana to generate tokens instead).
But after the chant resolved, the attachment effect resolved too. So now the equipment card is "destroyed", does not affect the wolf, and cannot be activated, but it is still visually attached to the creature.
