They care about the people to an extent but they're not exactly out there building roads and schools. Pay your taxes in the form if food and water and you'll have the BoS roll up to defend you when the raiders come knocking. Otherwise they'll make sure people dont blow themselves up with mininukes and clear out ruins of dangerous mutants and that's about it.

I judging them by how the BoS treats the wasters in the Commonwealth in F4. Like, they'll come protect you and all and make sure you can grow you corn un peace, but it's to make sure you can deliver the corn your promised to them on time.

They're a step bellow the NCR. With the republic you get a vote and representation. The NCR takes your taxes and uses it to build libraries and schools, help give you a better life. BoS makes sure you can pay their tithe without getting shot by raiders or ate by mutants.

Citizens of the NCR are citizens, wastelanders in BoS territory are subjects of the BoS, little more than medieval peasants.

This is said to be a huge contention that Cass states, as do other NPCs say as well.

Because the NCR is so big, they can't protect everyone, so they end up failing at protecting a lot - and a lot of the supply lines suffer because of that.

Thar's the other thing with the BoS, they're somewhat of a cult of personality. Without string leadership they falter and fall apart. There's no continuity, they loose their way.
