this happens because hovering over a mesh previews the mesh with the selected skin but doesn't make the "default" skin selection go away
so if you try to select a mesh but only hover over it and then hover over the "default" skin, you can get stuff like this

grabbers don't come from nowhere, they make a loud sound on spawn, if you was caught by grabber or killed by spitters then you are not skilled enough to sit outside alone, bulks are indeed not entirely under your control but least you can do is inform your team to get out of the bunker and use pheromones to significantly slow down the bulk and give your team a chance to regroup outside
