"comment vous allez ?" => informal, when addressing multiple people, or a teacher/business relation
"comment allez-vous ?" => formal, when addressing multiple people, or a teacher/business relation
"comment ça va ?" => informal, when addressing anyone

overall, the two important distinctions are:

• intonation-based questions for informal questions vs inversion for formal questions
• tu vs vous, a very important concept when addressing people
