J'ai mal à [article défini+ Partie du corps]
= My [body part] hurts.

Ex: J'ai mal au dos = my back hurts
J'ai mal aux pieds = my feet hurt
J'ai mal à la tête= my head hurts
J'ai mal aux épaules (fem.)= my shoulders hurt.

So in that case, "un mal" is an ache, an issue, that's why "un mal de tête" is a headache
