Make it descriptive, including relevant context, but also to the point. This way you improve your chances of getting a more relevant and specific answer.
« bien » is used to translate the word 'well' like « bien joué (well played) » in the sense that somethinge was done in a great and awesome manner. However, in this context, « bien » is used to emphasise the verb. In that sense, you can translate it with 'do/did' or with 'really'.
« Je l'ai entendu. (I heard it.) »
« Je l'ai bien entendu ! (I did hear it!/I really heard it!)'
"Bien entendu" on its own is a fixed expression, the "bien" essentially just meaning "well" as usual
