No, you need to change mon to ma when it precedes a feminine noun
(unless the following noun starts with a vowel sound)
You should look up the genders of each of the nouns in a dictionary (like wordreference.com)
It will say either (nm) or (nf) next to the noun indicating "nom masculin" or "nom féminin"
also you need to conjugate your verbs
The first phrase is also wrong
nom de famille not famille nom
Guessing it’s a straight translation from English ‘family name’
Also, what is « de base des français »?
Since age is a measurement, the sentence should be "mon âge est de XX ans" but we don't normally convey our age this way, we say "j'ai XX ans" instead.
Also, when you say "nationalité", you should rather give an adjective pertaining to the nation than a country name, so no "de la" or any other form.
