Make it descriptive, including relevant context, but also to the point. This way you improve your chances of getting a more relevant and specific answer.

• Que t'est-il arrivé ? (formal)
• Qu'est-ce qu'il t'est arrivé ? (standard, neutral)
  (There's also "Qu'est-ce qui t'est arrivé ?" when the construction isn't impersonal.)
• Il t'est arrivé quoi ? (informal)
  Se passer isn't used with a person or an object. You only use it with an event you want to know about.

Also, since se passer is a pronominal verb, the auxiliary will be être and not avoir.
