Make it descriptive, including relevant context, but also to the point. This way you improve your chances of getting a more relevant and specific answer.
I'm not good enough in English to know its translation, but Word Reference gives me "transcript of marks/grades" for "relevé de notes". A "relevé de notes" is an official document presenting the classes you took, your "success" in them (either your average grade for the course or the "cote de réussite"), and the amount of credits
