in English you would say "the first time that/when I spoke" since it refers to time but "quand" is a conjunction that connects two ideas not the same way "that" does

i'm having difficulty explaining it

but like "i was doing something when something happened"

the sentences with "question words" (où, quand, comment, etc.) should be able to be reworded as questions. in your example, you can't say "when did you speak?" "I spoke for the first time"

but you can say "la première fois que j'ai parlé était quand j'ai fait quelque chose"
