Nous sentons mal = We smell (as an action) badly
Nous nous sentons mal = We're feeling badly

« sentir » is the verb "to smell [something]"
« se sentir » is the verb "to feel like [something]"

basically "to feel oneself", because « se » is a reflexive pronoun, it means you apply that verb to yourself

And for « nous », the reflexive pronoun is also « nous », that's why you repeat it
