I suggest stopping the server, create a folder inside the plugin folder, and then one by one moving one of the plugins into it (stopping and starting each time)
Obviously start with the plugins that are most suspect to be causing issues
PlugMan does not disable/stop plugins and force a reload of files
So it is essentially useless to keep using it for troubleshooting

- I think TAB doesn't force lowercase (though it may) so you need to use lowercase group names.
- You're not using any placeholder from LP or vault to show the prefix at all, you're just manually adding it.
if you want the LP prefix to show, use %vault-prefix% iirc
or if you have PAPI and the LP expansion, use %luckperms_prefix%

You could get complicated with placeholderapi, but you don't really need to - the way you've set it up will work under two conditions:
- You make all those group names lowercase in the TAB config
- You make sure that you've set weights on the groups so a player's primary group will be the highest weighted and the format they get
