While it may sound like "hey I played this beta a while back" on the tin, what we're really looking for is any instances where you have played an early alpha/beta and actively reported bugs during that period.
If anything you've done in the past fits that bill, throw it onto the form.
If you've been placed in a game's credits for QA in the past as well, that's a big bonus. Definitely chuck that on there.
