So far, i've seen this happen with Havoc's Breath and Titanium Railgun (it may happen with other weapons too)
The only mods introduced are Calamity mod, Calamity mod music, and Dragon Lens (for testing).

@random haven please send a log file to make sure you dont have errors, mod conflicts and that your mods are up to date (i thought we fixed all of these instances what)
