Not sure what the repro on this is; however, I was downed, I was using a syringe and a teammate started to rev me. The teammate revive completed before the syringe revive, and about five seconds later, my player was turning into a zombie. (was on nightmare)
Edit: Issue has been resolved with Hotfix 2.0
