Hey there! Are you playing on Steam or Xbox? Also, does it happen only when Wand is active? In the meantime, try verifying/repairing your game files.
Hi team, I have encountered this issue for many times, too. Mine is Steam ver. and it only happens when Wand's active. This problem happens every time when any mod is activated in a previous mission.

Can you please take a **screenshot **showing the Build ID of the game? 
Step 1: Shut down Wand and Steam completely from your System tray, then run both as Administrator. Just right-click each app icon and choose "Run as administrator." If a pop-up asks for permission, click Yes.
Step 2: Verify your game files on Steam. Go to your Library, right-click the game, then select Properties > Installed Files > Verify integrity of game files.
Step 3: Try the mods again and let us know if the issue persists.
For checking the Build ID: Right clik the game in your library > Select Properties > Updates > Build ID at the bottom
