How do I trigger the teleport? It worked and then stopped working, I don't know what I did to make it work before.

If mod doesn't work on long- distance teleport, set a waypoint close to you then teleport again. If you get stuck in the ground or floating teleport to another nearby location. Set waypoint using space, not the middle mouse button.

What do I need to do to teleport? Open the map set a waypoint and then what? Press the right mouse click?

You can use the hotkey, the default is Ctrl + F11

Sure.
Still isn't working. I press TAB to open the map, place a waypoint with space bar, press right click on the mouse and it just exits the map and before it would teleport.

Once you place the waypoint, can you use the hotkey Ctrl + F11 instead of pressing right click?
