When I turn on "One hit kill" Dorian immediately goes from full health to dead as soon as I attack. He isn’t hit by anything. Hell he even dies if he drinks a health potion.

all wemod setting are good. Only mod's other than Wemod is a nexus hair mod

Can you test the one-hit kill without the hair mod? All of our trainers are designed and tested for the vanilla version of the game.

Ok so for a minute I thought it wouldn't make a difference since when I had the one hit kill turned on and varric took a potion he went from 3/4 health to dead... but then everything started to work... weird

Then in next fight I died instantly... theres no rhyme or reason it seems

Try to launch the game on Steam first and wait until you can move in the game before clicking Play. Test the One-hit kill again and let me know if there's any difference.
