I simply downloaded it from the forum, extracted and ran the exe

- Fully close WeMod first and make sure it isn't running
- Launch the bypass (also, make sure you've extracted it from the .zip file)
- Launch the game through Steam
- Once you've loaded into the main menu, launch WeMod again
- Go to the Phas trainer and click the Play button

The only way for you to be banned is if you used it on multiplayer and someone reports you, Then there is a chance.

Yes they do
You should avoid trying to max prestige which minimal hours on the game. Using a modified character on multiplayer can still get you banned.
