To enter safe mode on PS4, press and hold the power button. You will hear a first beep meaning it's powering on - keep holding until you hear the second beep.
PS4 will enter the safe mode, connect your DualShock 4 using a USB Cable (it won't work wirelessly) and select an option to change resolution
After the restart it will boot in 480p and will ask you to choose your resolution (choose automatic)
This process is the same on PS4 and PS5 however if you want to reset the resolution on PS3 do the same however it will just reset the video settings without going to the safe mode
